AI DISCLAIMER
Last updated May 12, 2026
Important: MyInvestigationCoach uses artificial intelligence to assist with workplace safety investigations. AI-generated outputs are provided for guidance purposes only and do not constitute legal, safety, or professional advice. You are responsible for all decisions made in connection with your investigations.
1. NATURE OF AI-GENERATED OUTPUTS
MyInvestigationCoach uses multiple third-party AI providers to generate investigation outputs including:
- Evidence checklists and collection guidance
- Incident timelines
- Root cause analyses
- Hierarchy of controls recommendations
- Investigation quality assessments
- Professional PDF reports
All AI-generated outputs are produced by large language models based on the information you provide. These outputs are starting points to support your investigation process — not definitive findings or conclusions. The specific AI providers we use may change from time to time. Our current data processing practices are described in our Privacy Policy.
2. LIMITATIONS OF AI
AI-generated content may contain errors, omissions, or inaccuracies. Specifically:
- AI can be wrong. Language models can produce plausible-sounding but incorrect analysis. Always verify AI outputs against your own evidence and professional judgement.
- AI does not know your full context. The quality of AI outputs depends entirely on the quality and completeness of the information you provide. Incomplete or inaccurate inputs will produce unreliable outputs.
- AI is not a substitute for expertise. Root cause analysis, safety recommendations, and regulatory compliance determinations should be reviewed by a qualified safety professional or legal counsel before being relied upon.
- Legislation references may be outdated. While we aim to provide current regulatory guidance, laws and standards change. Always verify legislative references against current official sources.
AI quality scoring does not certify compliance. The AI quality check feature measures the methodological rigour of the analysis process — it does not certify that an investigation is complete, accurate, or legally compliant. A high quality score does not mean an investigation is ready for submission to a regulator, insurer, or legal proceeding without independent review.
3. SERIOUS, NOTIFIABLE, AND FATAL INCIDENTS
Do not rely solely on this platform for serious or notifiable incidents. If you are dealing with a fatality, notifiable incident, or serious injury, you must engage a qualified safety professional and independent legal counsel before submitting any investigation documentation to a regulator, insurer, or legal proceeding. MyInvestigationCoach can support your investigation process — it cannot replace specialist expertise in high-stakes situations where regulatory, legal, and human consequences are significant.
If you are unsure whether your incident is notifiable, contact your relevant workplace safety regulator directly. In the US, contact OSHA at 1-800-321-OSHA (1-800-321-6742). In Australia, contact your state or territory WHS regulator.
4. YOUR RESPONSIBILITIES
By using MyInvestigationCoach, you acknowledge and agree that:
- You are solely responsible for verifying all AI-generated content before relying on it
- You are solely responsible for all decisions made in connection with your workplace safety investigations
- AI-generated outputs do not replace the need for qualified safety professionals, legal counsel, or regulatory advice where required
- You must comply with all applicable workplace health and safety laws, regulations, and standards in your jurisdiction regardless of what AI outputs suggest
- Any investigation reports generated by the platform should be reviewed by a competent person before being submitted to regulators, insurers, or used in legal proceedings
5. NOT LEGAL OR PROFESSIONAL ADVICE
Nothing produced by MyInvestigationCoach constitutes legal advice, safety certification, regulatory compliance confirmation, or professional engineering or medical opinion. The platform is a tool to support your investigation process — it does not replace professional advice.
6. LIMITATION OF LIABILITY
To the maximum extent permitted by applicable law, MySafeCoach's total liability to you for any loss, damage, or claim arising from or in connection with AI-generated outputs — including reliance on root cause analyses, recommendations, quality assessments, or investigation reports — is limited to the fees paid by you to MySafeCoach in the three (3) months immediately preceding the event giving rise to the claim.
MySafeCoach is not liable for any regulatory penalties, fines, legal costs, insurance claim outcomes, or consequential losses arising from your use of or reliance on AI-generated outputs. This limitation applies to the fullest extent permitted by law and does not exclude any liability that cannot be excluded under applicable consumer protection legislation, including the Australian Consumer Law.
7. AI PROVIDERS AND DATA
Your investigation data is processed by third-party AI providers solely for the purpose of generating investigation outputs. These providers process your data under their API terms and do not use your data to train their models. The specific providers we use may change from time to time. For full details on how your data is handled, see our Privacy Policy.
8. CONTINUOUS IMPROVEMENT
We continuously work to improve the quality and accuracy of AI outputs in our platform, including through cross-provider AI quality auditing, training example refinement, and prompt engineering. However, we cannot guarantee that AI outputs will be free from errors at any time.
